What to Expect
Your first visit to Village Oaks Dental is an important one as it introduces you to our practice and our staff. During the first visit, we will collect background information, medical history, complete a thorough examination, and get to know you and your dental needs. To help ensure your first visit goes as smoothly as possible, we invite you to take a few minutes to explore our website to educate yourself about some of the dentistry and smile solutions we offer.
In order to make your visit an easy process, you can request an appointment from our website or call 478-7201, our office. Our friendly staff will assist you in getting everything set up.
The patient forms below can be printed and completed before your first visit to expedite your check-in process:
We are in network with the following insurance companies:
- Delta Dental PPO
- MetLife PPO
- United Concordia
- Florida Combined Life; Blue Choice or Choice Plus, not “Copayment”
- Cigna PPO
- some United Healthcare (call our office to verify)
We will file with all other insurance companies on your behalf, but our unable to file with any Medicaid or Medicare plans at this time.
Payment is expected at the time services are rendered. We accept the following:
- American Express
CareCredit, a Synchrony Bank product, is a healthcare credit card specifically designed to pay for treatments and procedures your insurance doesn’t cover. It offers No Interest financing or low minimum monthly payment options so you can start treatment immediately and pay over time. You can even use CareCredit for your family and pets.
